Elementary School Enrollment
We wish to make your transition to our school as easy as it can be. Here's what to do:
Complete an Enrollment Application
Please visit our office to complete our Application for Enrollment and accompanying forms. You may follow this link to access and print out our enrollment packet to complete prior to your first visit to our office. Enrollment Packet - click here
Bring State-Required Documents with You
- Your child's original state-sealed birth certificate
- Proof of residency (current driver's license, purchase or rental agreement, a utility bill listing the registering address (not cell phone bill)
- Up-to-date Immunizations
- Kindergarteners will need proof of a vision and hearing screening as well as a health appraisal within the last year.
Visit the Elementary Office
Our school is located at 532 West Erie Avenue, Rogers City, Michigan, 49779. The office is located just inside the front doors and we are staffed from 7:30 AM - 3:30 PM throughout the school year. If you have any questions about the enrollment packet, we will be happy to help you when you visit.
